Hospitalist Program

The Hospitalist Group Will:

  • Assess and treat your medical condition
  • Coordinate your hospital care
  • Establish a relationship with you and your family
  • When discharged, your family physician will resume the responsibility for your ongoing outpatient treatment

Will I see other physicians while in the hospital?

The hospitalist group may consult specialists, i.e. cardiologist or gastroenterologist to assist in your care if necessary.

When you are discharged from the hospital?

A member of the hospitalist group will discuss your discharge medications and instructions with you.  He or she will assist in the scheduling of your first post-discharge office visit with your physician.  Your physician will receive a summary of your hospital stay, including discharge diagnosis, medications, and recommended follow up visits or treatment.
